Elamipretide in heart failure: no significant benefit in PROGRESS-HF

Level of evidence: Human randomized study
Research question
Does elamipretide lead to a greater change in left ventricular end-systolic volume than placebo in heart failure with reduced ejection fraction?
Study design
In this phase 2 study, patients were randomized to elamipretide or placebo. Heart volume was compared at baseline and after four weeks.
What was found?
The primary result showed no statistically significant difference from placebo. The publication therefore does not support a claim that elamipretide improved the cardiac function outcome studied in this trial.
Limitations and interpretation
The result applies to this population, treatment duration, and study formulation. A negative result for one endpoint does not mean that all mechanisms have been ruled out; nor should it be ignored in an overview of the evidence.
